SECSDA Permission Slip / Liability Form (This form MUST be filled out for each parent and pathfinder attending any events that occur off premise of Patmos Chapel building. It is a Souther Eastern Conference mandated requirement for everyone's safety. In addition, any adult attending a Pathfinder event is required to have a Sterling Background Check. Ask your club staff about this.)
