  • Please Note: it must be notarized and club fees paid in full at time of registration.

• SECSDA Permission Slip / Liability Form

  • This form MUST be filled out for each parent and pathfinder attending any events that occur off premise of Patmos Chapel building. It is a Southeastern Conference mandated requirement for everyone's safety. 2 Copies of the form should be filled out. 1 for the club director to have on hand. The 2nd for Southeastern Conference records. 

  • In addition, any adult attending a Pathfinder event is required to have a Sterling Background Check. Ask your club staff about this.

  • We take the safety of your Pathfinder/child seriously!
